This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. The Contractor shall initially place seventy (70), 20 gallon and thirteen (13), 96 gallon collection containers or compatible. This number may be adjusted up or downward as required with a one week notice. GLWACH IMD will determine the placement of collection containers based on waste volume. Locations of containers may be adjusted as necessary to handle dynamic work load but will be pre-approved by Contracting Officer prior to acceptance or placement. The Contractor shall provide paper waste removal with an onsite collection system. All containers shall be secured and access shall be restricted to the COR, the COR’s HIPAA representative, and the contractor only. GLWACH employees will not access waste once placed in the disposal containers. The contractor shall provide safe controlled handling of waste paper and provide certification of its destruction in accordance with applicable HIPAA regulation. Disposal service shall cover both the primary hospital facility and out buildings in accordance with the following table. Total weight per location is estimated based on 20lbs per 20 gallon container and 250lbs per 96 gallon container. All document destruction will be completed on-site, within the immediate vicinity of each pick-up location. Currently GLWACH maintains twelve (12) distinct locations that has been specified within the PWS. Disposal containers shall be supplied and placed by the Contractor within 30 days of contract award. The Contractor shall individually identify the containers by location and tracking number and their unique identifiers will be included in the tracking and reporting processes. Regular disposal schedule shall be bi-weekly for 24 weeks per year. One week minimum notice will be required for the government to suspend service for any postponed or canceled service. The Contractor shall furnish all materials, equipment, management, labor, supervision, training, and transportation (except as specified herein as Government furnished) to provide complete shredder services for General Leonard Wood Army Community Hospital (GLWACH). This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is W911SG-21-Q-0025 and is issued as a request for proposal. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-83 dated 03 Aug 2015. This requirement is 100% set aside for small businesses. The North American Industry Classification System (NAICS) Code for this requirement is 561990 Document Shredding Service is the NAICS with a size standard of $7 million. System for Award Management (SAM) applies to this procurement. You must be registered in SAM in order to be eligible to receive an award from this solicitation. Information on SAM registration and annual confirmation requirements may be obtained via the Internet at http://www.sam.gov. Prospective contractors must be registered in SAM database prior to award. Lack of registration in SAM database will make an offeror ineligible for award. Contractors must comply with FAR 52.204-99 System for Award Management (SAM) Registration (August 2012) (DEVIATION). The Mission Installation Contracting Command Fort Leonard Wood, MO 65473 intends to award a firm-fixed price type contract.
