(i) This is a combined synopsis/solicitation for a commercial item prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) This Request for Proposal (RFP) is being issued via solicitation number 75H70722R00001. (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisitions Circular 2021-07. (iv) Pursuant to the authority under FAR 19.5, this requirement is a Large business and only qualified offeror may submit bids and the associated NAICS code is 811219 with small business standard of $22.00. (v) The Contract Line Items (CLIN) and pricing structure are as shown below as follows: List should include materials used for repairs. (examples: turbine costs /labor prices). List should also include “rush” order services. PRICING: Offers will be accepted in the following format: FEE SCHEDULE OF SERVICES – ZCCHC (See attached) PAST PERFOMANCE FORM - (See attached) "Please include a minimum of 3 past performance surveys with your proposal from previous jobs similar in size and scope". (vi) The purpose of this firm fixed price is for dental hand piece repair services with the following terms: The Vendor shall provide various dental hand piece repair services for direct patient care for the Zuni Comprehensive Community Health Center (ZCCHC). BACKGROUND: The ZCCHC is to provide the highest quality of care to our patients, the Zuni Dental Department requires the use of many dental hand pieces to fabricate certain aspects of basic dental procedures including but not limited to simple restorations, complex restorations, Root canal therapy, fabrication of crowns, bridges, removable dentures and Oral surgery procedures. These are essential aspects of all dental operations and without an array of functioning dental hand pieces no dental office will be functional.
