PROVISIONS AND CLAUSES TO INCLUDE ADDITIONAL CLAUSES: 52.237-1 SITE VISIT (APR 1984) _______________________________________________________________________________________________________ This is a request for quotation (RFQ) for Janitorial Services for the Little Shell Service Unit, Little Shell Tribe Health Care Facility, 425 Smelter Avenue NE, Great Falls, Montana, 59404. This procurement is being conducted under Simplified Acquisition Procedures pursuant to the authority of FAR 13, Simplified Acquisition Procedures; and FAR 37.101, Nonpersonal Services Contract. The Little Shell Service Unit intends to award a firm fixed price purchase order to the contractor. The solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2021-07. The associated North American Industry Classification System code is 561720, Janitorial Services. B.1 PRICE SCHEDULE – PROFESSIONAL CLEANING SERVICES FOR THE LITTLE SHELL SERVICE UNIT TO INCLUDE CONSUMABLE SUPPLIES Monday through Friday 8:00am to 4:30pm 12 Months @: $_____________ Per Month Grand Total: $_____________ Consumable Supplies will be provided by the contractor and replenished as needed. Consumable supplies include but not limited to: Toilet Tissue, Hand Towels, Trash Liners, Hand Soap, and Hand Sanitizer. Proposals must be inclusive of transportation, meals, insurance, per diem and lodging. All federal holidays and overtime are not part of this request, at this time. The Government reserves the right to reschedule access to the site for cleaning to allow for meetings and/or other previously unscheduled events. Forty-eight (48) hours’ notice will be given to the contractor for all unscheduled events. This contract is subject to the Service Contract Labor Standards per FAR 52.222-41. Wage rates are included. B.2 PERIOD OF PERFORMANCE: Period of performance shall be for a 12 month period from date of award. B.3 BACKGROUND: The Little Shell Tribe was federally recognized in 2021 and subsequently entered in to a lease on a healthcare clinic to provide outpatient healthcare services to the population it serves. The clinic provides routine outpatient care to tribal members. B.4 OBJECTIVE / DELIVERABLES: The purpose of this contract is to award a contract for janitorial services. The contractor will perform the full range of custodial duties in assigned areas within the clinic. The building is approximately 8,900 square feet. Shall be knowledgeable and perform services using infection control practices in accordance with accreditation standards, IHS policy and procedures, OSHA, and other regulating agencies and laws. B.5 NON-PERSONAL SERVICES CONTRACT: This procurement is a Nonpersonal Health Care Service Contract, as defined in FAR 37.101, under which the Contractor is an independent contractor. The Government may evaluate the quality of professional and administrative services provided, but retains no control over the professional aspects of services rendered. The Contractor shall indemnify the Government for any liability producing acts or omissions by the Contractor, its employees and agents occurring during contract performance. B.6 SCOPE OF WORK The contractor will provide general janitorial duties in accordance with the listed job description below and replace/refill all cleaning supplies, toilet tissue, hand towels, soap/hand sanitizer, cleansers, etc. as needed. B.8 KNOWLEDGE Must have a basic knowledge of common cleaning solvents, cleaning tools, and equipment in a healthcare facility. Must know the proper uses of a variety of special cleaning and sanitizing solutions, and must be able to remove different kinds of stains from a wide variety of surfaces. Experience and training that furnish the knowledge, skills, and abilities needed to perform the duties of this position consistent with accepted practices of the trade. Ability to read, understand, and apply instruction and other materials related to the job. Work requires standing, stooping, bending, climbing, crawling, and kneeling for extended periods in uncomfortable positions. Worker may lift and move materials weighing up to 40 pounds and, occasionally heavier items with assistance. B.9 SUPERVISION. Contractor shall provide adequate personnel and supervision. B.10 ASSUMPTIONS Contractor will work with COR to obtain all the required security paperwork to start work. It is the responsibility of the COR and contractor to provide all the required documentation to begin work. B.11 CONTRACTING OFFICER AUTHORITY: Authority to negotiate changes in the terms, conditions or amounts cited in this contract is reserved for the Contracting Officer. B.12 CONTRACTING OFFICER REPRESENTATIVE (COR): The COR shall be responsible for: (1) Monitoring the Contractors technical progress, including surveillance and assessment of performance and recommending technical changes; (2) Interpreting the Statement of Work; (3) Technical evaluation as required; (4) Technical inspections and acceptance; and (5) Assisting the Contractor in the resolution of technical problems encountered during performance of this contract. B.13 INVOICE SUBMISSION AND PAYMENT: The Contractor shall submit its invoice to the Little Shell Service Unit. Contractor agrees to include the following information on each invoice: (1) Contractor’s name and address; (2) Contact name, title, and telephone number; (3) DO/TO/PO/Contract Number; (4) Invoice number and date; (5) Description, quantity, unit of measure, unit price, and extended price of supplies delivered or services performed; (6) Remit to Address; (7) Taxpayer Identification Number (Employer Identification Number or Social Security Number); and (8) Data Universal Numbering System (DUNS) number (to ensure payment is made to the correct bank account) and vendor mailing address, as provided in the System for Award Management (SAM), formerly known as the Central Contractor Registration (CCR) database. Payment shall be made by the Billings Area Financial Management Branch, P.O. Box 36600, Billings, Montana 59107. B.14 PRO-CHILDREN ACT: The Contractor certifies that it will comply with the provisions of Public Law 103-227, Pro-Children Act of 1994, which imposes restrictions on smoking where federally funded children's services are provided. B.15 CHILD CARE NATIONAL AGENCY CHECK AND INVESTIGATION (CNACI): A CNACI must be completed for all Indian Health Service (IHS) contractor personnel within the Billings Area. Public Law (P.L.) 101-630, Indian Child Protection and Family Violence Prevention Act and P.L. 101-647, Crime Control Act of 1990, require the IHS to conduct a character and criminal history background investigation on all contractors performing services in IHS facilities. Fingerprints must also be taken as part of the pre-employment process and must be completed before the provider is allowed to work. B.16 SPECIAL CONTRACT REQUIREMENTS: Contractors cannot serve as expert witnesses in any suit against the Federal Government. Many of the IHS patients receiving services may only speak a native language and/or reside on a Native American Reservation, therefore, the Contractor must demonstrate sensitivity to cross-cultural and language differences. The Privacy Act of 1974 mandates that the Contractor maintain complete confidentiality of all administrative, medical and personnel records, and all other pertinent information that comes to his/her attention or knowledge. The Privacy Act carries both civil and criminal penalties for unlawful disclosure of records. Violation of such confidentiality shall be cause for adverse action. IHS shall provide training on the Freedom of Information Act and the Privacy Act. All IHS regulations and policies applicable to these Acts shall be enforced. The contractor shall comply with IHS facility infection control and safety procedures, practices, and standards. B.17 PROPOSAL SUBMISSION INSTRUCTIONS: In order to be considered for award, contractor must (1) provide quotation for all items listed, (2) provide copy of three contracts similar in scope to this service. Contractors intending to conduct business with the Federal Government must register with the System for Award Management (SAM).
