VISN 19 - Custom Sterile Surgical Packs / Custom Procedure Trays (CPTs) The purpose of Request for Proposal (RFP) 36C25919R0099, Amendment 0005 is to; Extend the Proposal Submission Date shown on Pg. 1 (SF1449) and Pg. 46 (Instructions to Offerors). The Proposal Submission Date is hereby changed from Nov 16th, 2021 to Nov 30th, 2021, 4:00pm (EST). Provide the Government's answers to the questions submitted by the RFP questions cut-off date. Provide the updated SF1449 Request for Proposal attachment with answers to questions addressed. Section B.2 Price/Cost Schedule has been updated to reflect future POP dates set to begin Jan. 8th, 2022.
