The Federal Bureau of Prisons, Field Acquisition Office, Grand Prairie, TX, intends to issue solicitation 15B20521Q00000029 for the provision of Mortuary Services. The services will be provided to individuals who are under the custody and care of the Federal Bureau of Prisons within a secure correctional environment, located at the Federal Medical Center, Devens, NC. The physical address is Federal Medical Center (FMC), 42 Patton Road, Ayer, MA 01432. The contract type is an indefinite/delivery requirements contract with firm-fixed unit price. The award will be based on past performance and price. The contract periods for the service will consist of a base year with an effective date on March 6, 2022 through March 5, 2023 with four 12-month option years for renewal. The forthcoming Statement of Work will provide all the details regarding the required services. See the attached list for line items and quantities needed. The solicitation will be available on or about Nov 5, 2021 and will be distributed solely through the beta SAMs website at www.SAM.gov. All future information concerning this acquisition, including solicitation amendments, will be distributed solely through this website. Interested parties are responsible for monitoring this site to ensure you have the most up-to-date information about this acquisition. Hard copies of the solicitation will not be available. The website provides all downloading instructions to obtain a copy of the solicitation. The anticipated date for receipt of quotes is on or about Nov 30, 2021. Faith-Based and Community-Based organizations can submit offers/bids/quotations equally with other organizations for contracts for which they are eligible. All contractors doing business with the Federal Government are advised that this future solicitation includes the clause FAR 52.212-4(t) for Commercial Items and System for Award Management (SAM). This clause requires all contractors doing business with the federal government after September 30, 2003, to be registered in the SAM database at www.sam.gov. Contractors should include their Data Universal Number System (DUNS) number in their offers. The Contracting Officer will verify registration in the System for Award Management (SAM) database prior to award by entering the potential awardees DUNS number into the SAM database. The acquisition is set-aside for Total Small Businesses. The North American Industrial Classification (NAICS) code is 812210 with a small business size standard of $8 million. Schedule of Items: Contract Line Items as described below constitute services, equipment, and supplies to be provided by the Contractor under the terms of this contract 001: Death Certificate: 4 certified copies per death 12 each 002: Body Preparation: (embalming, cosmetics, etc.) 002a- Normal Case 9 each 002b- Autopsied Case 3 each 003- Clothing: (Conservative colored suit of clothes with a tie and all necessary undergarments, socks, and shoes). 12 each 004- Professional Services: Mortuary Services include, but are not limited to, staff to respond to initial request for services; preparation and filing of necessary certificates and permits, recording vital statistics; coordination with those providing other services, cemetery, crematory, and others. 12 each 005- Caskets: (as specified in the Statement of Work) 005a- Wood Casket 1 each 005b- Standard Metal 12 each 005c- Metal with Hermetic Seal 1 each 005d- Oversized Casket 3 each 006- Transportation: Contractor to arrange for all air freight to ship the body to the designated funeral home. The government is to be billed the actual shipping cost supported by a copy of the air freight bill. This may also include transportation costs from receiving airport to receiving funeral home WHEN requested and supported by invoice from same. 006a- Price to be based on a per mile rate: Rate may apply to special cases where air connections are difficult or very expensive or time is critical. 2,000 each 006b- Pick-up and Delivery: Usually from contracted outside medical facility. In these cases, the Contractor will be responsible for providing a facility, having an autopsy performed, and all associated costs. This line item will not apply if the autopsy is remanded by the Commonwealth of Massachusetts Medical Examiner. 3 each
